Annual financial status - bank request

Hi

I have received a letter from Caisse d'Epargne bank asking for my financial status, a "formulaire 5000" - all in french - for both me and the wife.  Has anyone got a simple answer to how to provide this?

I have banked with Caisse d'Epargne for 20 years and have never received such a form. I suggest to just visit the closest branch and ask what it is all about. But first make a quick Googgle translation to have an idea of what it is about. If you are outside of France, give them a call; they almost always have somone who understands English.Bonne chance.
